A free statewide service for people affected by cancer, providing support, information and linking with patient services.
Metropolitan area
The metropolitan cancer nurse coordinators facilitate the continuity and quality of care of patients diagnosed with cancer. Their roles vary according to the tumour type for which they are responsible and the complexity of care required by patients. They are responsible for the coordinated approach of cancer services rather than the case management of patients, integrating with private services when required.
They are the only health professional with a whole system view of the patient’s journey and therefore will be responsible for improving communication between all the parties of the consumer care pathway.
It is the responsibility of the metropolitan cancer nurse coordinators to bring the tumour collaborative best practice guidelines to the clinical floors. They also develop the cancer knowledge base of other nurses and have a statewide influence in the specific tumour group.
Rural services
The primary role of the rural cancer nurse coordinators is to improve the coordination of care between local and metropolitan services to ensure that patients have supported access to the treatment options. They are the link in each region for the coordination of all services and are establishing strong links between all health professionals.
There is a rural cancer nurse coordinator based in Perth who facilitates the transition of care for rural cancer patients with complex coordination needs.
These nurses have a regional remit.
