Pathways and protocols implementation

This program aims to have pathways and protocols adopted as normal practice and included in the monitoring systems of each health workplace.

Liverpool Care Pathway for the Dying

The Marie Curie Palliative Care Institute (MCPCI) in Liverpool, England has used end of life care pathways for over 10 years and is internationally regarded for this work. The Liverpool Care Pathway (LCP) is a quality framework which supports a high standard for care of the dying. The Palliative Care Network registered with the MCPCI to modify the LCP for use in Western Australia and conducted a pilot study in Perth (PDF 235KB).

The LCP will be implemented over the next three years commencing in the Midwest, South West and Great Southern regions of the WA Country Health Service. The implementation program will be evaluated.

Community medications project

The purpose of this project is to:

  • develop and implement palliative care medication protocols
  • standardise availability of palliative care medications
  • establish links between regional community pharmacies and palliative care teams, with additional support available from metropolitan specialists
  • develop and implement strategies for medication management by terminally ill people and their families living in the community
