Models of Care

Health Networks have developed more than 60 evidenced based models of care and policy frameworks that provide a plan for health care across WA. A report on the implementation (PDF) of the models of care and frameworks shows there has been significant progress made.

WA Framework for Diabetes and Diabetes Service Standards

In an Australian first, WA Health through the efforts of its’ Diabetes and Endocrine Health Network has released the WA Framework for Diabetes and Diabetes Service Standards 2014 (PDF). The Network has also developed foot care standards for people with diabetes (PDF) to supplement the framework.
