Improving health services - model of care implementation
Models of Care have been developed in collaboration with a broad range of stakeholders and outline a plan for improving health services in Western Australia. Health Networks are working with their partners across the health sector to translate these models into improvements in service delivery.
In late 2009 Health Networks started capturing information on projects that are already underway that support the recommendations of the Models of Care already developed. The current full reports on these projects is available now (see related links box).
WA Health has also set up a strategic group, the Model of Care Implementation Review Committee to drive and monitor the implementation of the Models across WA Health. Their role includes the prioritization of Models of Care for implementation as well as providing a statewide forum to report and evaluate the progress of implementation. The Committee includes representation from Area Health Services, WA GP Network, Health Consumers’ Council, Network Leads and other key WA Health divisions.

Todate Health Networks have received information on more than 120 projects covering part of all of Western Australia that support Models of Care. School Breakfast Program (Model of Care: WA Child and Youth Health Framework)A healthy breakfast is provided to students at registered schools in metropolitan, rural and remote WA. Crunch&Sip® (children's fruit, vegetable and water consumption campaign) (Model of Care: WA Child and Youth Health Framework)Crunch&Sip® is a set break in the classroom where children eat fruit and vegetables and drink water every school day. Schools become certified after developing a policy that is endorsed by the whole school community. The project aims to increase the consumption of fruit and vegetables and water in primary school aged children. It also educates and promotes the importance of eating fruit and vegetables to children and aims to instil healthy lifestyle behaviours at a young age. |
The WA Draw the Line (healthy weight) campaign (Model of Care: Diabetes and Cardiovascular-related Models of Care)Healthy weight social marketing mass media campaign and associated support strategies. Aimed at adults aged 22 years and above state-wide to engage consumers in a healthier lifestyle. Specifically to prevent unhealthy weight gain and thereby reduce coronary disease, cancer and type 2 diabetes. |
Aboriginal Women's Project (Model of Care: Diabetes, Heart Failure, Stroke, Colorectal Cancer)The project provides culturally appropriate health promotion information, activities and support to Aboriginal women and their families. The focus is on the benefits of physical activity, healthy diet, healthy weight and smoking cessation. |
FOODcents® (healthy eating and budgeting program) (Model of Care: Diabetes, Colorecal Cancer, Heart Failure)The Foodcents® program teaches people about how to get value for money from their shopping budget while achieving a healthy balanced diet. Organisations run community workshops on nutrition and food budgeting based on the Foodcents® principles. |
Go for 2&5® campaign (adult fruit and vegetable consumption) (Model of Care: Diabetes, Colorectal Cancer, Heart Failure)A comprehensive social marketing campaign targeting fruit and vegetable consumption. In line with the national dietary guidelines, the campaign recommends eating 2 serves of fruit and 5 serves of vegetables every day for better health. |
