Chronic Obstructive Pulmonary Disease (COPD)

The COPD Model of Care recommended a COPD Action Plan for people with COPD that contained information on key health service providers, current treatment regime and what to do in an emergency. A wallet-sized card has been produced along with patient information and health information fact sheets. The Action Plan is being distributed to patients by medical specialists, community based health service providers, GPs and peer support groups.
