Cystic fibrosis model of care
The cystic fibrosis model of care (PDF, 548KB)
The model of care proposes the establishment of a statewide cystic fibrosis (CF) service. It was developed in consultation with clinicians, key stakeholder groups, consumers and carers. It takes a holistic consumer centred approach focusing on:
- improved transition from paediatric to adult services
- expanded community-based services provided by a skilled multi-disciplinary team
- increased consumer participation
A small working group is now working on the implementation of the model. They will focus on the following recommendations from the model of care, aiming to:
- develop referral pathways with an emphasis on access to multidisciplinary, community-based care, where possible supported by telemetry
- develop a road map for transition across developmental age groups with a particular focus on the transition from adolescence to adulthood
- develop evidence based guidelines and protocols
- develop workforce strategies to provide integration, reorientation and capacity building to improve consumer access to primary care and community-based services
The implementation working group consists of clinicians, Area Health Service planners, primary care representatives, community-based organisations, consumers and carers.
